RLR Creations Sewing Guest Tester Application Form

Please answer as accurately as possible.  It is important to have a range of sewing experiences on the team so please be honest. 

Testing lasts from a week to two weeks based on the project being tested. If selected for the upcoming test I will notify you as soon as possible to see if you are available. There are times during a test when unforeseen things come up and a tester needs to drop out. If this happens I may need to give less notice.
